We do not participate with any commercial insurance plans

We are Medicare providers for traditional Medicare patients receiving pelvic floor therapy only

If you carry commercial insurance, you may be eligible for out-of-network reimbursement by submitting your receipts directly to your insurance provider. However, payment is required in full at the time of service.

By completing your online booking, you acknowledge and accept our private pay status and agree to pay at the time of your appointment.

Dr. Tara Kazemfar PT, DPT, PRPC

Born and raised in Crystal River, Florida, Tara discovered her passion for helping people move and thrive early on. She earned her undergraduate degree in Exercise Science/Pre-Physical Therapy at Florida A&M University and her Doctorate in Physical Therapy from the University of Florida, completing specialized rotations in neuro and sports physical therapy.

Tara is the only Herman & Wallace certified pelvic floor physical therapist in the LC Valley, a distinction that reflects her advanced training and dedication to this highly specialized field. She established the valley’s first comprehensive pelvic floor therapy program and has since been focused on caring for individuals with pelvic pain, as well as prenatal, postpartum, and pediatric populations.

At RegenTech Physical Therapy, she brings an UNcommon approach to pelvic health—combining specialized hands-on care, progressive movement strategies, and advanced regenerative technologies such as laser therapy, shockwave, and multi-waveform stimulation—to help patients Regenerate. Restore. Rise. Her goal is to empower patients to regain strength, improve function, and reclaim the activities that matter most.

When she’s not in the clinic, Tara enjoys fishing, hunting, jiu-jitsu, and kickboxing, fully embracing the active lifestyle she encourages in her patients.

My career began as a basketball coach, where I learned firsthand the value of strategy, consistency, and helping people unlock their full potential—lessons that form the foundation of my approach to physical therapy.
